During your assessment, we evaluate how your loved one moves through their home and completes everyday tasks. We identify opportunities to improve safety, comfort, and independence while respecting the life and routines they value.

YOUR HOME INDEPENDENCE ASSESSMENT INCLUDES

CAREGIVER CONSULTATION

Before your visit, you will complete a Caregiver Pre-Assessment so we understand your concerns, goals, and what changes you may already be noticing.

90-MINUTE IN-HOME ASSESSMENT

During our visit, we will evaluate the home environment and observe how your loved one interacts with their space.

We look at:

Entry and exterior access
Mobility throughout the home
Bathroom safety
Bedroom setup
Kitchen functionality
Laundry routines
Lighting and visibility
Trip hazards and barriers
Daily living tasks

FUNCTIONAL TASK OBSERVATION

When appropriate, we may ask your loved one to demonstrate everyday tasks:

Carrying laundry
Preparing a meal
Entering/exiting the shower
Moving through frequently used spaces

This allows us to identify challenges that may not be obvious during a conversation.
